Series consists of freedom of information request case files received by the Ministry of Education, and other ministries, between 2000 and 2002.
During this period some ministries shared information and privacy offices and this series includes records from the Ministry of Education, Ministry of Advanced Education, and Ministry of Advanced Education, Training and Technology.

The files consist of freedom of information requests that were investigated by the Office of the Information and Privacy Commissioner (OIPC). Files also include a copy of the original application for records, correspondence between staff and the applicant, forms documenting the status of request, correspondence between the ministry and the OIPC, submissions to the OIPC, correspondence with the Legal Services Branch, and a copy of the OIPC order. These files also include photocopies of the requested pages that show those portions that were severed prior to release.

The files are arranged by a file number that starts with year and ends with a chronological number for each year’s request. Each number also includes an acronym that is based on the ministry responsible for the request. The following acronyms have been used as part of the file number: EDU (Ministry of Education), AET (Ministry of Advanced Education, Technology and Training) and AVE (Ministry of Advanced Education). All files were classified as 292-30 in the Administrative Records Classification System (ARCS).

Series consists of case files of freedom of information requests received by the Ministry of Employment and Investment and deemed to be significant by staff. This series also includes the case files for two requests that were appealed to the Office of the Information and Privacy Commissioner.

The requests consist of a wide range of information requests relating to the ministry’s mandate. The files include a copy of the original application for records, correspondence between staff and the applicant, and correspondence among ministry staff. These files also include photocopies of the requested pages.

Files are arranged in a sequential numerical system based on the order that the request was received by the ministry. There are large gaps in the number system since these records are a sample.

The files in this series were classified as 292-30 in the BC Government Administrative Records Classification System (ARCS).

Series consists of case files of freedom of information requests received by the Office of the Premier between 1994 and 1995. All files consist of freedom of information requests in which the response was appealed by the applicant to the Information of Privacy Commissioner for review of the decision.

The files in this series usually consist of correspondence between staff and the applicant, a copy of the original application and forms documenting the status of request, correspondence between the ministry and the Office of the Information and Privacy Commissioner (OIPC), submissions to the OIPC, correspondence with the Legal Services Branch, and a copy of the OIPC order. The files also include photocopies of the requested pages, some of which have been highlighted to show what has been severed in a document.

Series consists of a freedom of information request received by the Williams Lake District Office of the Ministry of Forests in 1996. The l request was for copies of cutting permits and road permits for the years 1995-1996. Following a complaint, the Information and Privacy Commissioner’s Office acted as a mediator between the ministry and the applicant.

There are 21 files in the series. Files 1 and 21 consist of correspondence between staff and the applicant, correspondence between ministry staff, a copy of the original application to view records, and photocopies of the requested pages which have been severed. Files 2-20 consist of copies of the requested pages.

The series has been classified with a file number that includes the year followed by a sequential number. The file has also been assigned the number 292-30 in the Administrative Records Classification System (ARCS). Although the ministry’s centralized Information and Privacy Branch assigned these numbers to all requests, it was the responsibility of regional offices to respond to the applicant.

Series consists of case files of freedom of information requests received by the Ministry of Aboriginal Affairs in 1996. This series includes the case files for three requests that were appealed to the Office of the Information and Privacy Commissioner.

The files consist of a copy of the original application for records, correspondence between staff and the applicant, correspondence between the ministry, correspondence and consultations with third parties and the Office of the Information and Privacy Commissioner (OIPC), submissions to the OIPC, and correspondence with the Legal Services Branch. These files also include photocopies of the requested pages which include highlighting of the restricted documents. Most of the requested documents date from 1993-1996.

Files are arranged in a sequential numbering system based on the order that the request was received by the ministry. The three files are numbers 65/96, 66/96 and 72/96. The files in this series were also classified as 292-30 in the BC Government Administrative Records Classification System (ARCS).

Series consists of case files of freedom of information requests received by the Ministry of Transportation, and the Ministry of Transportation and Highways, between 1993 and 2006.

The files in this series usually consist of correspondence between staff and the applicant, a copy of the original application and forms documenting the status of request. The files also include photocopies of the requested pages. Some of these photocopied pages have been highlighted to show what has been severed in a document. These photocopied pages usually consist of correspondence and plans that were originally created between 1915 and 1994 which were filed in the ministry’s central operational files.

The files are arranged by a file number that includes the year followed by a sequential number. Pre-1994 files, which predate the October 1993 proclamation of the Freedom of Information and Protection of Privacy Act, are arranged by the name of the applicant. All files were also classified as 292-30 in the BC Government Administrative Records Classification System (ARCS).

The series includes two subseries. The first subseries consists of case files that have been deemed to be significant by government staff (accessions 92-7795 and 92-9594). The requests consist primarily of applications by First Nations for information about road and highway right of ways through First Nations land.

The second subseries consists of the ministry’s case files for Information of Privacy Commissioner investigations (accessions 91-3384, 92-9542). These files are similar to the rest of the series but they also contain correspondence between the ministry and the Office of the Information and Privacy Commissioner (OIPC), submissions to the OIPC, correspondence with the Legal Services Branch, and a copy of the OIPC order.

Series consists of ministry copies of freedom of information requests that were investigated by the Office of the Information and Privacy Commissioner (OIPC). The files reflect the work of one government information and privacy office and that of the OIPC. There is also one file that represents a judicial appeal about one of the orders of the OIPC.

The freedom of information requests were processed by one Information and Privacy office that represented multiple ministries and related agencies. The files document requests for records from the Ministry of Environment, Land and Parks, Ministry of Water Land and Air Protection, Ministry of Sustainable Development, and Ministry of Environment, between 2000 and 2007.

The files consist of a copy of the original application for records, correspondence between staff and the applicant, correspondence between the ministry and the Office of the Information and Privacy Commissioner (OIPC), submissions to the OIPC, correspondence with the Legal Services Branch, and a copy of the OIPC order. These files also include photocopies of the requested pages. The files in this series were classified as 292-30 in the BC Government Operational Records Classification System (ORCS).

Ministry staff assigned a number to each of these files. Most files were assigned a number that consisted of a ministry prefix followed by a sequential number. Pre-2001 file numbers were assigned a number that reflected the year of the request followed by a sequential number.

Freedom of Information requests : human blood and tissue files

The series consists of files collected by the Ministry of Health Services to fulfill Freedom of Information requests in 2003. The records were pulled from numerous accessions held in a variety of offices across the province. Although the files were compiled in 2003, they often include copies of records that were originally created in the 1980s and 1990s. The records address several issues pertaining to human blood, tissue and cell collection, including leukemia and bone marrow treatments, budgetary and financial issues, and organ transplants in infants. The series consists of reports, correspondence, internal memos, and meeting minutes and agendas.

The records are covered by ORCS 25600-20 of the Health Authority Performance Management schedule (schedule 122352) and are arranged according to subject.

Information and Privacy Commissioner investigations

The series consists of Information and Privacy Commissioner investigations resulting from freedom of information requests from citizens of BC to the Ministry of Children and Family Development and its predecessors on matters such as adoption, children in foster care, allegations of abuse in the home, and support services for children requiring additional care. The request for access to records and subsequent investigations were conducted by the Information and Privacy/Records office, Ministry of Children and Family Development (2001-) and its predecesors; the reviews were conducted by the Office of the Information and Privacy Commissioner (OIPC). Following the completion of the freedom of information request, the applicant may ask the OIPC to review decisions to release, not release, or sever records made by the ministry. This series of records consists of a copy of the original access request, correspondence between staff and the applicant, correspondence between the ministry and the OIPC, submissions to the OIPC, and a copy of the final report. The files often contain intake reports and packages, investigation and interview notes, acknowledgement of receipt of records forms, and copies of materials such as criminal records, social worker handwritten notes, child protection checklists, adoption files, medical records, and judicial records.

The records are arranged by applicant surname and are classified as either family support services or adoption services. The records include copies of correspondence, memoranda, reports, information packages, copies of affidavits, checklists, and legal files. The records in this series were classified as 290-20 in the BC Government Administrative Records Classification System (ARCS). Ministry staff also assigned an additional number that consisted of the year of the investigation followed by a sequential number.

Information and privacy investigations

The series consists of copies of freedom of information requests, or privacy complaints, which resulted in an investigation. The investigations were conducted by the Ministry of Health and in some cases by the Office of the Information and Privacy Commissioner (OIPC). All files were created by the Information and Privacy office the representing the Ministry of Health (2006-2008) and the Ministry of Health Services (2009).

The files consist of a copy of the original application for records, correspondence between staff and the applicant, correspondence between the ministry and the Office of the Information and Privacy Commissioner (OIPC), submissions to the OIPC, and a copy of the final report. Some files also include photocopies of pages requested by the applicant.

The files in this series were classified as 290-20 in the BC Government Administrative Records Classification System (ARCS). Ministry staff also assigned an additional number that consisted of the year of the investigation followed by a sequential number.

Miscellaneous files relating to freedom of information

Series consists of miscellaneous files created by the Information and Privacy unit of the Ministry of Attorney General. Although most of the files relate to the Ministry of Attorney General, the unit also provided services to the Ministry of Public Safety and Solicitor General and there are files that reflect this.

Records consist of policy and procedures files, correspondence, reports, training material, meeting files, reference material and copies of investigation reports. The investigation reports document privacy investigations by the ministry, or the Office of the Information and Privacy Commissioner. Some of these investigation files relate to other government bodies and were kept for reference purposes.

Files are arranged by the classification number taken from the Administrative Records Classification System (ARCS).
