Public and preventative health records

The series consists of public and preventative health records relating to the development of programs and policies for the provision of prevention services directed at persons and groups who are at risk for developing a health problem, disease or disorder. These programs are guided by the Health Act (RSBC 1996, c. 179); some programs may also fall under the Health Act Communicable Disease Regulations (BC Regulation 567/98), and the Dentists Act (RSBC 1996, c. 94). This series includes records relating to ensuring best evidence policy and program development and maintenance, community health services issues pertaining to cardiovascular disease, communication disorders, musculoskeletal disorders, neurological and behavioral disorders, nutrition issues, and health prevention and promotion; and health services for mentally challenged adults living in the community. Some of the programs/projects documented in these records include ActNow BC, Dial-a-Dietician, BC heart health project, Pregnancy Outreach program, and Action Schools!. These records were created and received by the Population Health and Wellness Division, Ministry of Health between 1987 and 2006 in BC.

The records are arranged alphabetically by project or program, or by issue or title. Record types include correspondence, reports, presentations, briefing notes, research materials, research data, financial statements, program/project planning materials, drafts, notes, and workshop/outreach materials. These records are classified under section 3 public and preventative health of the Community Health Programs ORCS (schedule 122353), 2011. The files are assigned an alphabetical acronym code and the following ORCS numbers: health promotion and prevention projects, programs, initiatives, and strategies (62330-30); non-communicable disease project and program files (62390-30); nutrition project files (62420-30); nutrition program and initiative files (62420-40); and communication disorder hearing and audition projects (62250-30). The series also includes annual, operational, research, statistical, and other reports (62420-45). Some files have been removed for tobacco litigation. Gender reassignment surgery human rights complaints

This series consists of human rights complaints related to gender reassignment surgery from 2005. These records were created by the Ministry of Health, and its predecessors, in response to complaints filed with the Human Rights Tribunal.

Records relate to the assessment and decisions regarding requests for Medical Services Plan (MSP) funding for gender reassignment surgery (GRS). Gender reassignment surgery requires pre-authorization for MSP funding. The beneficiary’s physician refers the request to MSP, where the application is assessed by Medical Services Branch staff, the consultant psychiatrist, and the Gender Reassignment Surgical Review Committee, who issues the letter of decision. This includes the resulting human rights tribunal case and actions resulting from the decision.

The series includes records of one case regarding discrimination based on sex resulting from different coverage provided to individuals undergoing male to female, and female to male transitions, as described in the records.

Record types include correspondence, memorandum, reports, briefing notes, copies of complaint, investigation and resolution records, financial records and the decision document of the BC Human Rights Tribunal.

Health Authorities committee files

This series consists of committee files from the Health Authorities Division and its precursor, the Performance Management and Improvement Division from 2002-2007, which included the following branches: Evaluation and Strategic Directions Branch, Health Authority Branch, Home and Community Care Branch, Mental Heath and Addictions Branch and Provincial Branch.

The committees in this series are high level province wide committees with representatives from the Ministry of Health, the Provincial Health Services Authority, the regional health authorities, academic institutions, and non-government health service and planning organizations. Ministry of Health representatives may include the deputy minister, assistant deputy ministers, and management level staff.

Many of the committees report to the BC Health Leadership Council, which is chaired by the Ministry of Health Deputy Minister. Most of the committees have a mandate that involves defining strategies and priorities in particular areas of the provincial health care system (e.g., surgical services, critical services, emergency services, eHealth).

Records include minutes, agendas, terms of reference, agreements, correspondence, survey, reports and other reference materials utilized in committee meetings.

And the General Practice Services Committee (GPSC): a joint committee between government and the BC Medical Association to support and encourage family practice medicine in BC. Prevalent issues relate to chronic disease management, obstetrics and providing adequate income to general practitioners.

Medical Services Plan strategic program and initiative records

This series consists of Medical Services Plan strategic program and initiative records and other health committee files created by various divisions of the Ministry of Health and Ministry of Health Services from 1995-2011. Many records may have been created or used by the Medical Services Commission.

Records relate to medical and health care issues, studies or strategies initiated by or affecting the Medical Services Plan (MSP) with respect to the provision of physicians’ and health care practitioners’ services. This includes the development, planning, implementation, and management or maintenance of strategic programs and initiatives to support and enable the provision of health services to all residents of the province. Strategic programs and initiatives include primary health care initiatives, rural health initiatives, physician workforce planning or physician human resource management, physician utilization, chronic disease management, acute care programs, and continuing care programs.

Records include briefing notes, correspondence, committee minutes, contracts, budgets, reports, presentations, financial records and statistics. The series may also include related committee files and executive records.

This series is divided into the following subseries based on the program or group that created the records:

  1. Medical On-Call Availability Program (MOCAP), 2000-2009.
    MOCAP was created to provide unassigned or orphaned patients access to physicians and specialists for on-call emergency medical services. Records also include several other programs for doctor coverage and payment to improve medical care across the province by the Medical Services Plan (MSP). These records are related to physician resource planning or physician human resource management.

  2. Rural health records, 1995-2006.
    These records include those of the Joint Standing Committee on Rural Issues (JSCRI) and other committees related to healthcare in rural areas. The JSCRI was responsible for implementation of the subsidiary agreement for physicians in rural practice to enhance availability of stability of physician services in rural areas by addressing relevant unique and difficult circumstances. This subseries also includes committee files for the Mental Health Evaluation and Community Consultant Unit and Medical Services Commission Group Management.

  3. Blood and Laboratory Services, 2003-2009.
    Files address several issues related to blood and laboratory services, including BC Lab transformation, lab services reform, a telepathology project and studies of lab benchmarks.

  4. Chronic Disease Management and Primary Health Care Renewal, 2003-2011
    These records were primarily used by the Chronic Disease Management and Primary Health Care Renewal unit and its successor Primary Health Care. Most files are of committees related to General Practice doctors and managing chronic diseases, such as diabetes.

  5. Other files regarding a variety of topics, 2001-2009.
    These records include a variety of programs, services and committees, most related to health sector workforce and negotiating compensation for healthcare services. Some committees include Fee-for-Service-Transfers Review Team, Physician Strategic Services Advisory Committee, Emergency Medicine Committee, Electronic Medical Records Working Group, and Physician Information Technology Office Steering Committee. Prominent subjects include emergency room services and training, and electronic medical records. Records include committee files, Implementation reports, two Alternative payments agreement accountability records case files, and Subsidiary agreement dispute resolution.

Aboriginal Healthy Living records

This series consists of records of the Aboriginal Healthy Living group and related records created from 1993-2010. Records include committee files, executive records, treaty negotiation records related to health issues, and aboriginal health education and research files.

Committee files include correspondence, presentations, terms of reference, records of decision, and other records. Committee files relate to the Provincial Aboriginal Social-Economic Strategy, Aboriginal Health Transition Fund Adaption Committee, Aboriginal Health Leads Committee, Aboriginal Health Network, Aboriginal Health Transition Fund Workshop Subcommittee, and Aboriginal Reference Group Meetings.

Executive records include briefing notes, referral replies and issues records.

Treaty negotiation case files relate to the negotiation of health related agreements with First Nations outside of the B.C. Treaty Commission process and the negotiation of formal pre and post treaty agreements. Records may relate to the identification and negotiation of the Ministry's responsibilities for the delivery and provision of certain health programs and services, the definition of new authorities for First Nations over the design and delivery of health programs and services, and the establishment of inter-ministerial and/or inter-governmental agreements. Files relate to Nisga’a pre-treaty agreements related to MSP and treaty negotiations, as well as the ʔaq̓am (St. Mary’s Indian Band) Community Care Home interim agreement and Maa-Nulth First Nations Treaty Society negotiation.

There are also Aboriginal health research project files on: First Nations Chief Health Committee Terms of Reference, the Special Cabinet Meeting on August 17, 2002 with First Nations Summit, and Status Indian Health Utilization database.

Alternative Payments program files

The series consists of applications, reports, forms, correspondence, printouts from government databases, and budget information received, maintained and created by the Ministry of Health and its successors. The records were created between 1968 and 2011 for the Alternative Payments program. The series documents the provision and monitoring of Medical Services Plan (MSP) funding for physician and health care practitioner services through alternative payment agreements to regional health authorities. The functions of the Alternative Payment program were controlled by the Physicians’ Compensation branch.

The Alternative Payments program is a method of funding physician services by the Medical Services Plan through means other than the fee-for-service method. The program is designed to secure sufficient access to care in situations where fee-for-service arrangements may not guarantee physicians the financial support or stability to be able to provide needed care, such as in teaching hospitals or community-based services in rural areas. There are two models for alternative payments : service agreements and sessional arrangements. Service agreements consist of a contract between the Ministry of Health and a Health Authority which retains physicians under contract or direct employment. Sessional arrangements are based on contracts between a Health Authority and a physician.

Records consist of alternative payment agreement accountability records case files. Records regard reviewing applications from regional health authorities for funding for programs within their geographic areas of responsibility; developing and monitoring agreements with regional health authorities; monitoring programs funded by alternative payments methods; and processing payments to regional health authorities in accordance with approved agreements. The series also include some files with related policies, procedures and forms.

Information and privacy investigations

The series consists of copies of freedom of information requests, or privacy complaints, which resulted in an investigation. The investigations were conducted by the Ministry of Health and in some cases by the Office of the Information and Privacy Commissioner (OIPC). All files were created by the Information and Privacy office the representing the Ministry of Health (2006-2008) and the Ministry of Health Services (2009).

The files consist of a copy of the original application for records, correspondence between staff and the applicant, correspondence between the ministry and the Office of the Information and Privacy Commissioner (OIPC), submissions to the OIPC, and a copy of the final report. Some files also include photocopies of pages requested by the applicant.

The files in this series were classified as 290-20 in the BC Government Administrative Records Classification System (ARCS). Ministry staff also assigned an additional number that consisted of the year of the investigation followed by a sequential number.
