The Royal Commission of Inquiry into the escape of prisoners from the Lower Mainland Regional Correctional Centre was approved and ordered by Order in Council number 1/1988 on January 6, 1988 under the Inquiry Act, section 8.
The Honourable Judge Ian L. Drost of Vancouver was appointed Commissioner with directions to inquire into and report on all matters and circumstances leading up to the escape of 13 prisoners from the O.S.U. (Old Segregation Unit) of the Lower Mainland Regional Correctional Centre in the early morning of January 1, 1988. In particular the Commissioner was to investigate and report on: the cause and reason for the escape from the Centre; the effectiveness of the facilities at the Centre for the containment of prisoners; methods for setting shifts and systems for shifts at the Centre; security and reporting procedures employed by officers on duty at the Centre and any matter considered by the Commissioner to be relevant to the containment of prisoners at the Centre.
The Commissioner held sittings from January 29 to June 28, 1988 and presented his report on September 29, 1988. In it he concluded that there were no specific reasons for the escape. However the causes of the escape were identified as the failure by management to appreciate the fundamental insecurity of the O.S.U.; the lack of clear and consistent directions by management and certain senior staff officers regarding the operations and staffing of the secure unit; the lack of personal alarm transmitters, and a disturbing lack of vigilance on the part of some of the staff officers who worked in the unit.
Scope and content
The series consists of records created by the Royal Commission of Inquiry into the escape of prisoners from the Lower Mainland Regional Correctional Centre in 1988.
Records include Commissioner's (Honourable Judge Ian L. Drost) report, lists of exhibits, witnesses, days of sitting, counsel appearing and the minutes of proceedings. In addition there are 70 exhibits, which include Commission administrative records such as a certified Order in Council appointing Drost as Commissioner, his oath, notice of public hearings and Gazette notices as well evidence presented to the Commission such as statements, policy and procedure and training books, photographs and drawings of the grounds, cells and individuals involved in the event, two videotapes of previous prison riots and three brass rods.
The series also contains 67 volumes of the transcript of proceedings which took place from January 29 to June 28, 1988.
